Intermediary abutment

Regardless of the production method (wax up or CAD-CAM), if the implants are set very deep (more than 2mm) under the tissue it becomes necessary to place intermediary abutments specially designed for screw retained restorations. These abutments are already placed by the restorative dentist and impressed with special impression copings or the laboratory will select appropriate collar height abutments for screw retained restorations. The UCLAs as well as the analogs for screw retained abutments are designed differently than UCLAs that screw directly to the implant or fixture level analogs and are not interchangeable.

1. Wax and cast

Non indexing UCLAs

Bar type overdentures can be waxed and cast with non engaging UCLAs as a Hader® bar, Dolder® bar or with attachments (Locator® by Zest Anchors, O-ring, SwissLok a.o.) on the frame. UCLAs are either manufactured with a machined metal abutment to implant interface and a plastic tube processed to the metal interface or all plastic. Some manufacturers machine the metal interface out of titanium or a gold alloy. The gold alloy interface will result in a better cast on bond than a titanium interface. A all plastic UCLA is considerably cheaper than a gold plastic combination but it sacrifices the precision fit of machined UCLAs. Choose appropriate platform diameter UCLAs from the implant manufacturer or clone parts. Even severe implant angulation divergence can be compensated with a bar set up. Vertical clearance needs to be carefully evaluated to leave enough room for the denture acrylic.

2. CAD-CAM

Verification index

Modern CAD CAM technology enables the manufacture of titanium bars machined from one piece titanium disks with much the same attachment configurations as a waxed bar but without the purchase of UCLA abutments or metal cost. As with a waxed bar the use of intermediary abutments may be indicated. The abutment to implant interface is machined in a non engaging fashion. A verification index made with non engaging metal temporary abutments is required since it is almost impossible to laser weld a titanium bar. The verification index needs to be tried in the patients mouth to ensure the working model is exactly the same as the patient situation.

CAM StructSURE® bars for overdentures can be milled as Dolder®, Hader® and primary bars for a large variety of implant brands. Currently there are only options to have the bar pre taped for a wide selection of attachments. The attachments then need to be ordered from a third party ( Attachments International )  and installed by the laboratory. CAGENIX™, AccuFrame®

AccuFrame® bars can be milled as Dolder® and Hader® bars. A Locator® bar is available but the cost for the Locator® attachments is an extra charge. The list of supported implant brands is limited and most have to be machined with intermediary abutments. Dentsply Compartis® ISUS™

Compartis® ISUS™ bars can be milled out of titanium and chrome cobalt as Hader® or Dolder®bar and a small selection of attachments is available for bar construction. Attachments are charged extra. Biocad produces the most beautiful finished titanium bars for the widest selection of supported implant brands. Hader® and Dolder® bars are possible. Laboratories with Procera® capabilities are able to virtually design the bar in house after sending the master model to Biocad. Any attachment milled on to the bar is included in the price.
